Before submitting this form, please review the following:

  • Ensure that the correct side is being used — the right earpiece is marked “R”.

  • For decilo SLEEP & RELAX earplugs, an adaptation period of 1–2 weeks is recommended. If discomfort occurs, it is advised to wait before submitting this form, as replacing the product too early may reduce sound isolation. In case of severe pain, use should be discontinued immediately.

  • Verify that the 5-digit code on the product matches the patient ID indicated on the packaging label.

  • Adjustments and manufacturing defects are covered under warranty. However, due to the fully custom nature of the products, refunds are not available.
